Wavier Of Liability, Assumption of Risk, Indemnity Agreement 
By signing I agree that I understand and accept each of the terms set forth below and will abide by them. 
 
I further acknowledge that I am participating at my own risk. 
 
Furthermore, I hereby understand:

• Cancellation Policy: rescheduling is okay with a minimum 14 day written notice. No refunds.
• All classes and materials will be paid in advance of beginning work- this applies to classes, custom orders and special projects.
• Your class or party fee is due at time of booking.
• Agree that by providing your email and phone number, you agree to receive emails and text messages related to your order and may receive promotional emails and offers from us. Standard text messaging rates may apply. Reply STOP to opt-out of receiving future text messages related to your order or unsubscribe from any email sent to you.
• I authorize the use of my photograph for, but not limited to publications on the internet, magazines, journals, books, social media, etc. provided that it is done for lawful purposes.  Upon the usage of my photographs, I consent to such materials becoming the sole property of Lucie Art llc and I will no longer be entitled to them provided that it is done for lawful purposes.  I hereby release all rights to any but not limited claims, rights, demands and/or any causes of action by me or my representative, heirs or anyone else.  I waive my right to any royalty or any compensation with regard to the usage of the photos referred to in this form.  I waive the right to approve any photos prior to usage.
• On behalf of myself, my personal representatives, heirs, children and dependents, next-of-kin, spouse and assigns, I acknowledge that I give up certain legal rights when I sign this document which includes the right to recover damages to personal property, bodily injury, and death as a result of my activities in Lucie Art LLC (LALLC / Releasee) programs.
• Release, discharge and promise not to sue LALLC doing business under its own name or any other name and/or any of its owners, officers, employees, instructors, and agents (hereinafter the ‘Releasees’) for any loss, liability, damage or cost whatsoever arising out of or related to any loss, liability, damage or injury (including death) to my person or property.
• Release the Releasees from any claim that such Releasees are or may be negligent in connection with my educational experience, including but not limited to instruction on glass working techniques, maintenance or care of glass working tools and equipment.
• Indemnify, and save and hold harmless the Releasees from any loss, liability, damage, injury, or cost that may arise out of or in any way connected with my use of glass and any of the tools and equipment provided or any acts or omission of Releasees, or other employees, volunteers, teachers or agents.
• Acknowledge that this document is a contract and agree that if a lawsuit is filed against or its owners, agents, employees or instructors for any injury or damage in breach of this contract, the Signor will pay all attorney fees and costs incurred by LALLC in defending such action.
• Voluntarily assume the risk and danger of injury or death inherent in the use of the equipment and tools provided by LALLC and its instructors.
• By registering or attending any class or event I and all dependents expressly agrees that the foregoing release and wavier of Liability, Assumption of Risk, and Indemnity agreement is governed by LALLC.
• I agree only to use the equipment, machinery or engage in a process that I have been instructed on and been approved to use in the safe and proper use of the studio instructor. I agree that I will not use any equipment, machinery or take place in any process without an instructor or monitor present unless given permission to do so by the class instructor.
• Acknowledge that there is an inherent danger in working with glass at various temperatures, and that attending LALLC programs involves risks that may cause personal injury or death.
• I agree to return all equipment I use to its rightful place in good working condition.
• Agree to abide by, follow instructions and rules established by LALLC or any of its instructors, agents or employees with regard to safety operations in using glass and glass working tools or equipment.
